Pyrexia of Unknown Origin—A Case Study

چکیده

Tuberculosis (TB) is often considered in the differential diagnosis of pyrexia of unknown origin (PUO) and miliary tuberculosis (MTB) a common cause of PUO. MTB is a potentially lethal disease if not rapidly diagnosed and treated, but remains a potential problem as the presentation may be nonspecific, and unusual associations may be overlooked. A high index of clinical suspicion, early diagnosis, and timely institution of antituberculosis treatment is life-saving. We describe a case of MTB that presented with pyrexia of unknown origin with atypical presentation causing a delay in diagnosis.

Two cases of pyrexia of unknown origin are described in which no cause was found despite exhaustive inpatient investigation until occult dental infection was detected: extraction of the teeth involved was followed by resolution of the pyrexia. Dental infection should be considered as an unusual but eminently treatable cause of pyrexia of unknown origin.
